NCCI requests <br />approval of Technical Revision No. 5. <br />Item No. 1 - Submit an updated Mining Plan Map showing all of the ground water monitoring well <br />locations. Attached you will find a Mining Plan Map showing ten (10) monitoring well locations. <br />Monitoring well No. 2 has not been monitored since April of 2009. This monitoring well is located <br />on the east side of WCR 25 and is not located on the mining property. Since April of 2009 this <br />property has be fenced off and the gate locked. As of April 2011 we have made arrangements to <br />access the monitoring well and it will be monitored on a monthly basis from now on. Monitoring <br />well No. 9 was taken out of service in April 2009 when we installed a recharge ditch along the North <br />side of the large mining area. We will not be installing a replacement monitoring well in this location <br />in the future. Monitoring well No. 10 was taken out of service in March of 2010 when we relocate an <br />irrigation ditch for the property to the North of the main mining area or the large water storage area of <br />our gravel mine. We plan to install another monitoring well in the same area by June of 2011. This <br />will provide a total of nine (9) monitoring wells for the gravel mine. <br />Item No. 2 - Submit all past ground water monitoring data. Attached you will find all past ground <br />water monitoring data from January 2007 to the present. <br />Item No. 3 - Submit benchmark depths for ground water as approved under TR 03. Attached you will <br />find benchmark depths for ground water. This information is part of the ground water monitoring <br />data. <br />Item No. 4 - Submit a report demonstrating ground water depths are being maintained within two feet <br />of the benchmark depths. If measured ground water depths have a variance of greater than two feet, <br />then the report should include a plan for restoring ground water levels to the benchmark depths. Early
