PERMIT #: M-2011-004 <br />INSPECTOR'S INITIALS: BMK <br />INSPECTION DATE: February 15, 2011 <br />OBSERVATIONS <br />1) The Division conducted a pre-operational inspection of the site as part of the normal Division's requirement to <br />conduct such inspections when a new or an amendment application is submitted for a 112 permit. Present during <br />the inspection were Dr Angela Belantoni the operator's consultant and Mr. Michael Karuth, the operator. <br />The site is located approximately 1 mile east of Rio Grande Road/Garden Park Road, north of Canon City. The total <br />proposed acres under the current permit application are 35.05 acres. All of the current proposed acres to be affected <br />were covered under a previous permit # M-1998-030. The Division released the site in August 2006, after a site visit. <br />No reclamation associated with the previous operation in the per law ares was noted at the time of this inspection. <br />Fragments of sandstone slabs were noted scattered throughout the proposed site, left from the pre- law disturbed <br />area. operator. There are two access roads to the property; both these roads are proposed to be left in place when <br />the mining operation is completed, to allow future access to the property. The current operator plans to utilize <br />Rubber Tired skidsteer or rubber tired excavator, to remove the sandstone outcrops. The estimated size of the <br />deposit is approximately 5 acres and 20 feet deep. There are two dry gulches that run through the proposed deposit <br />area, operator plans to obtain a general storm water permit from CDPH&E. Operator plans to utilize some the sand <br />stone slabs as temporary berms along these drainages as part of the storm water BMP plan for the site. There is very <br />little top/soil or growth medium in the area. However, operator has committed to salvage the entire available <br />growth medium in the proposed affected area. Pre-mining and post mining land use is Range land. <br />2) The entire proposed affected and permit area was inspected. The permit area markers were properly posted. Permit <br />application sign was also posted at the access to the property. No manmade structures that were not identified in <br />the application were noted during the inspection.
