PERMIT #: M-1997-087 <br />INSPECTOR'S INITIALS: BMK <br />INSPECTION DATE: February 15, 2011 <br />OBSERVATIONS <br />1) The Division conducted a monitoring inspection of the site as part of the five year inspection program. Present during the <br />inspection was Mr. Rusty Cochran of Lafarge Inc. <br />2) The site is a phased, 112 sand and gravel operation that has been in reclamation since 2007. The total permitted acres is <br />240. Forty acres of the total permitted acres are privately owned and the rest owned by the State Land Board. All the <br />mineral rights are owned by the State. <br />3) On 1/2/2007, Lafarge Inc received approval of technical revision # 1, from the Division. The revision allowed the operator to <br />leave in place, approximately % acre of processed stockpile area, with the existing processed stockpile in place and the <br />access road, from further reclamation responsibility. Both the State Land Board and Mr. Hartman, the mineral & surface <br />lease holder agreed with all the terms of the revision approval. <br />4) The entire reclaimed acres were inspected. Even though seeded in 2007, very little grass growth was noted. The operator <br />plans to have the entire area seeded again this spring or fall. No noxious weed infestation was observed at the time of the <br />inspection. No major erosion features noted in the reclaimed slopes. The entire permit area is fenced, but no permit <br />identification sign was noted at the access to the property. The lack of permit sign is noted as a possible problem. Operator <br />plans to place the permit sign and submit a photo as proof no later than March 30, 2011. <br />5) The current reclamation warranty for the site is $139,285.00. It is more than adequate to complete the remaining <br />reclamation task. The operation is in compliance with the approved reclamation permit. <br />Page 2 of 4 <br />
