April 25,2011 C-1994-082/Yoast Mine SLB <br /> Haul Road A was saturated(upper 2"of the road surface was liquefied)and impassable beyond Pond 010 <br /> to the north pit. No other roads were open. When conditions allow,the operator will need to maintain ditches and <br /> culverts along Haul Road A,particularly the outlet of Culvert YA6,where significant downcutting was occurring. <br /> SLIDES and DAMAGE -Rule 4.12: <br /> The reclaimed areas immediately above Ponds 013 and 014 were accessible on foot. Two small areas of <br /> instability were observed in the reclaimed area above Ponds 013 and 014. <br /> The feature above Pond 014 was a crack approximately 125' long, 24-32"deep vertically,with a scarp width of <br /> approximately 6-8". It is located at 4471526.85N,314169.9E. <br /> The feature above Pond 013 was approximately 200' long,vertically offset by 4-5', and 20' wide at the scarp. <br /> Material had rotated from the scarp and bulged at the toe of the feature. The feature extended from approximately <br /> 4471731.45N,314155.39E to 4471691.18N,314203.04E. This area failed and was repaired in 2009. <br /> A long crack was observed running E/W along the YPM 17 drainage, approximately 100'long, 3"wide, and 6-12" <br /> deep. <br /> Two slide areas were observed in the previously repaired area above Pond 012. Approximately 300 linear feet of <br /> the previously repaired slide on the north slope above Pond 012 has moved again, and approximately 75 linear feet <br /> at the base of the shrub reestablishment area,parallel to the upper end of YPM 6-3 had slumped,with an <br /> approximate 3' headscarp. Neither of these features was accessible by vehicle or on foot; the upper 4-6"of the <br /> surface was completely saturated and unstable. <br /> When conditions allow, Seneca Coal Company will need to repair all of the unstable areas. <br /> SIGNS AND MARKERS—Rule 4.02: <br /> Permit identification signs and boundary markers were observed at the Haul Road A entrance from Routt <br /> County Road 27 and at the entrances to Ponds 012, 013, and 014 along Routt County Road 37. <br /> Number of Partial Inspection this Fiscal Year: 8 <br /> Number of Complete Inspections this Fiscal Year: 3 <br /> Page 3 of 8 <br />
