Box 187 <br />Craig, CO 81626 <br />RE: Trapper Mining, Inc., Permit C-1981-010 <br />First Quarter 2011- Horse Gulch Fill Quarterly Inspection Report <br />Dear Mr. Mattern: <br />The Colorado Division of Reclamation, Mining, and Safety (CDRMS) has reviewed and found the above referenced <br />report to be in compliance with the following CDRMS regulations: <br />4, 09. ] (1 ])(a) Inspected at least quarterly by a reregistered engineer or other qualified professional specialist in <br />construction of earth and rockfill embankments under the direction of a qualified engineer <br />throughout the construction <br />The certified report covers the first quarter monitoring period from January to March 2011, and the <br />inspections were conducted by Seth Puls, a certified engineer. <br />4.09. 1 (11) (b) Provide to the Division a certified report within 2 weeks after each inspection that the fill has been <br />constructed as specified in the design approved by the Division, The report shall include a <br />description of any appearances of instability, structural weakness, and other hazardous conditions. <br />The inspection was conducted on March 30, 2011, and the report was received by the Division on <br />April 7, 2011. No instability, structural weakness, or other hazardous conditions were noted during <br />this inspection. <br />4.09. 1 (11) (c) Any certified report addressing the underdrain system and protective filters shall include color <br />photographs taken during and after construction, but before underdrains are covered with excess <br />spoil. If the underdrain system is constructed in phases, each phase shall be certified separately. <br />...photographs shall be taken of the underdrain as the underdrain system is being formed. The color <br />photographs accompanying each certified report shall be taken in adequate size and number with <br />enough terrain or other physical features of the site shown to provide a relative scale to the <br />photographs and to specifically and clearly identify the site. <br />The report includes no photographs of the underdrain system and related construction activities. <br />This is acceptable because construction was complete in the first quarter of 2011, and no additional <br />construction activity is anticipated. <br />No problems or maintenance items were noted during the inspection of the Horse Gulch Excess Spoil Fill. <br />If you have any questions feel free to contact me.
