Pineda <br /> Director <br />DINS,-,Eon o' Raciarriation, <br />Kning and Safety <br />Re: Willow Creek Mine, Permit/File No. M-1983-011, Receipt of Hard Rock/Metal Mining Regular (112) Operation <br />Reclamation Permit Conversion Application Package (CNi) <br />Dear Mrs. Bellantoni: <br />On Friday, April 08, 2011, the Division of Reclamation, Mining and Safety received the Hard Rock/Metal Mining Regular <br />(112) Operation Reclamation Permit Application package for the Willow Creek Mine, Permit No. M-1983=011. A <br />preliminary review of the information received determined that the following items/information must be received before <br />the Division can consider the application submitted and technical review can begin: <br />Per Rule <br />6.4.5 EXHIBIT E - Reclamation Plan <br />(c) A description of how the Reclamation Plan will be implemented to meet each applicable <br />requirement of Section 3.1; <br />Describe how the Reclamation Plan will be implemented to meet Rule 3.1.9. The Plan should include the proposed <br />locations and configurations of topsoil stockpiles. If topsoil is to be stockpiled for periods greater than one year, the <br />Plan should describe the means to be employed in order to prevent loss of topsoil caused by wind and/or water erosion. <br />(f) A description of each of the following: <br />(iii) Fertilization - if applicable, specify types, mixtures, quantities and time of application; <br />Inform the Division of the type of fertilizer (if any) and amount per acre to be used during reclamation at the site. <br />Office of Denver • Grand Junction • Durango Office of <br />Mined Land Reclamation Active and Inactive Mines
