Reclamation Permit Conversion Application Package (CNi)
Dear Mrs. Bellantoni:
On Friday, April 08, 2011, the Division of Reclamation, Mining and Safety received the Hard Rock/Metal Mining Regular
(112) Operation Reclamation Permit Application package for the Willow Creek Mine, Permit No. M-1983=011. A
preliminary review of the information received determined that the following items/information must be received before
the Division can consider the application submitted and technical review can begin:

6.4.5 EXHIBIT E - Reclamation Plan
(c) A description of how the Reclamation Plan will be implemented to meet each applicable
requirement of Section 3.1;
Describe how the Reclamation Plan will be implemented to meet Rule 3.1.9. The Plan should include the proposed
locations and configurations of topsoil stockpiles. If topsoil is to be stockpiled for periods greater than one year, the
Plan should describe the means to be employed in order to prevent loss of topsoil caused by wind and/or water erosion.
(f) A description of each of the following:
(iii) Fertilization - if applicable, specify types, mixtures, quantities and time of application;
Inform the Division of the type of fertilizer (if any) and amount per acre to be used during reclamation at the site.
