PERMIT #: M-1977-342 <br />INSPECTOR'S INITIALS: PSH <br />INSPECTION DATE: April 20, 2011 <br />OBSERVATIONS <br />The Henderson Mine, File No. M-1977-342, was inspected by Peter Hays and Tony Waldron with the Division <br />of Reclamation, Mining and Safety (Division) as a follow up inspection to the notice by Tim Haynes with Climax <br />Molybdenum of a release of tailing slurry from the tailing delivery line at Drop Tower 7. Mr. Tim Haynes and <br />Mr. Miguel Haramat with Climax Molybdenum (Climax) were present during the inspection. <br />Climax is required by Rule 8.1(b) to notify the Division of a failure of any Environmental Protection Facility <br />(EPF) designed to contain or control process solution. The tailing pipeline and the tailings slurry are <br />considered an EPF and process solution in the permit. Mr. Haynes informed the Division of the tailings <br />discharge on March 18, 2011 by phone. The leak occurred for approximately 1.5 hours and release <br />approximately 90,000 cubic yards of slurry. The slurry flowed under County Road 3, along the pipeline to <br />Bearpaw Gulch and through a culvert which connects the gulch to the tailings pond. No tailings slurry left the <br />affected area boundary. <br />During the inspection, the Division observed the erosion caused by the tailing flow at the base of Drop Tower <br />7, along the pipeline which created a ditch and in Bearpaw Gulch. The cause of the pipeline/drop tower <br />failure was being completed during the inspection. Climax personal initially thought the failure occurred in the <br />floor of the drop tower, however after cleaning and inspecting the tower Climax determined the problem was <br />caused by a hole in the concrete pipe after it exited the drop tower. The plan was to repair the pipeline with <br />concrete. Climax committed to provide a follow-up report to the Division as required by Rule 8.2.3.
