PERMIT #.M-1980-244
INSPECTOR'S INITIALS: BMK
INSPECTION DATE: Februarv 1 T. ?011
OBSERVATIONS
1) The Division conducted a monthly monitoring inspection and quarterly water•sampling from the Arequa'.GW
mcinitoring/compl,iance wells and the under drain discharge locations. Present during the water sampling inspection was
Mr:`.Lawrence Myers. Present during the monitoring inspection of the valley leach facility `were" Mark Vanoni;& Timm
Comer from CC&V'and Joe H, ickey of Amec.
2) After.'purging the wells, the required four casing volumes,. a spilt sample-was collected for sulfate and wad cyanide`from
compliance well, CRMW 3A.&3B. Additional split"samples were. collected from•the valley leach( under. drain discharge, AG -1
and south under:drain. All lower ponds and AG 1.5 were dry. All required. field measurements were recorded prior to
collecting samples: Samples collected were delivered to the Colorado Department of Health and Environment.
3) : Continued the inspection of-the Phase V valley leach facility. area,:which is still under-construction: Th'e.pregnant pond has
been-certified, and is'being.loaded,with.ore. Barren, solution is being. applied to the pond, to lower pond.volumes in phases
1 Il,and IV ponds. The ore storage area construction is expected to commence by early April, or sooner if`the weather
permits. All the temporary storm water diversion structures in place, delineating the lined and.unlined areas, of the Phase V
ore storage ar"ea;,were found to be in good repair. The following readings were collected from the.Phase V, high and Low
volume solution collection transducers. High Volume Solution Collection Transducer # 312 was at:17.2, #313'was at 17.29,
# 314;16.82 feet of hydrostatic head. Low volume solutions collection transducer# 1 was at 9.82 and # 2 .was at 9.9 inches of
hydrostatic head.,No,discharge from the low"volume solution discharge pipe was noted at the time the inspection. In fact
according to Timm Comer and Mark Vanoni, there had notbeen`a discharge from the pipe since application of barren
solution"to the pond,was approved:..
4). Continued the inspection to the existing valley leach facilities'l,ll,11, & IV. They, were acfive.at the time of the inspection:
Storm.water drainage controls forthe facilities were found to be in good repair: Inspected the.Phase IVC perimeter ditch. It
Y Was-foundto be in good repair. While'in the area collected the following readings from the High and low volume solution
collection; transducers for Phase I;II,II & V. Phase I high volume solutioncollectior transducer # T.was recorded at 27.1,# 2
was at 34.2`and #3.was'at31.9..Low volume solution collection transducer #1.was at 0.37 and #:2 was at 0:50: The two
piezometers for the pond were. recorded at 43.3 & 42.4 feet. Phase II and 111 High'Volume Solution" Collection Transducer fl
4.was recorded at 22.5, #5W as at 20.3 and # 6.was at 22.5. Low volume solution collection,transducer # fwas;at 0.09 and
#:2 was at 0.30. Piezometerfor the pond was recorded at 31.9.' Phase. IV higKvolume solution collectiontransducer'# 7 was
at44A # 8 was at 42.9, 09 was at 48.7 and # 10 was at,40 2. Low volume solution collection transducer# 1 was recorded at'
13:9 and # 2 was at,9 3-, inches of hydrostatic head:;All other transducer readings are in feetof hydrostatic head. Inspected
the external pond, it was holding water. One of the low volume solution collection pressure transducerreadout was not
- functioning; Number II was recorded at 11.2 inches of hydrostatic head. CC&V plans to replacethe:readout meter as soon
. ' as possible ; '
5). Inspected the"Cresson pit area. All the surface water diversion structures were found to be in good repair. Active.mining
wasaaking place in both the main and south Cresson.pits,.at the time of :the inspection. Inspected the clay borrow area. It
was actively processing-clay in"anticipation of resumption of the phase V valley leach ore storage area construction. Storm
water drainage control for the borrow area were found to be in, good repair.
