PERMIT #: M-1983-194 <br />INSPECTOR'S INITIALS: THM <br />INSPECTION DATE: April 5, 2011 <br />OBSERVATIONS <br />This inspection was conducted by the Colorado Division of Reclamation, Mining and Safety in response to a <br />spill report provided by Natural Soda, Inc. on March 31, 2011 via phone message and a written spill report <br />received April 4, 2011. <br />The spill occurred during a pressure test of an inactive pipeline located approximately 1,000 feet south of <br />Natural Soda's process facility. The cause of the spill was the failure of an old paper type gasket at a pipe joint. <br />Evidence of spilled barren process brine was observed adjacent to the inactive pipeline. <br />The spill path was a small surface drainage that directed the spilled fluid to the northeast for approximately <br />300 feet. The amount of fluid spilled was approximately 1,000 gallons with an estimated 500 gallons <br />recovered. <br />The small drainage affected by the spill was cleaned by hand tools and approximately a 1 inch depth by 3 feet <br />wide along the drainage bottom of contaminated soil was recovered and disposed of properly. <br />The extent of the spill damage was minimal.
