PERMIT #: M-2005-077 <br />INSPECTOR'S INITIALS: THM <br />INSPECTION DATE: April 8, 2011 <br />OBSERVATIONS <br />The Upland Gravel Pit #3 was inspected by the Division of Reclamation, Mining and Safety (DRMS) as part of <br />the normal inspection and monitoring program. Scott Gerwe with the Bureau of Land Management (BLM) <br />Grand Junction Field Office was also inspecting the site due to the operation being located on both private and <br />federal BLM land. Jason Rinderle of Upland Gravel, Inc. (Operator) accompanied the inspectors during the <br />inspection. <br />The focus of the inspection was to observe the changes to the access road required by MSHA. According to <br />the Operator widening of the mine access road was required immediately in order for the operation to avoid a <br />mandatory shut down for non-compliance with MSHA safety standards for road construction. The Operator <br />also stated that the road was not built narrower than the approved maximum width. According to the <br />approved Mine plan the road's maximum width is approximately 28 feet wide. The conditions observed <br />during the inspection are in compliance with the approved Mine Plan. <br />Due to the severe wind occurring during the inspection a complete inspection was not conducted. The DRMS <br />will conduct a follow-up inspection once all modifications to the road are complete.
