DECISION MEMORANDUM AND CATEGORICAL EXCLUSION <br />U.S. Department of the Interior <br />Bureau of Land Management <br />Dolores Public Lands Office <br />COSO4000 <br />Categorical Exclusion # DOI-BLWCO-S04-2011-0064 <br />DENISON MINES MONITORING WELLS INSTALLATION <br />1.0 DECISION <br />This Decision approves the drilling of three monitoring wells at Denison's Sunday-Topaz mine <br />complex. The purpose of these wells is to collect environmental information pertaining to <br />groundwater quality and depth. This is being approved as a minor mine plan modification for <br />the Topaz Mine (COC-52049), and assumes all applicable stipulations, mitigation measures and <br />other requirements applied to that mine plan or any successors. <br />Drilling would be initiated as early as mid-April 2011, and would continue for approximately L' <br />months. Drilling may however be initiated later if circumstances dictate, and may take longer if <br />unexpected circumstances are encountered. Drilling should be done before December 1, 2011. <br />Total disturbance from road construction and well pads is estimated at less than 2 acres. The <br />access to the wells may be left open for the life of the mine and until the wells are no longer <br />needed for monitoring purposes, at which time they must be reclaimed according to applicable <br />State of Colorado well abandonment requirements. <br />Detailed lithologic, mud, gamma, and other applicable logs typically kept during monitoring well <br />installation will be kept and submitted to BLM and DRMS. <br />This decision becomes valid as soon as a bond adequate to cover the reclamation of the new <br />disturbance is calculated by the Colorado Division of Mining Reclamation and Safety, and funds <br />in that amount are contributed by Denison and accepted by DRMS. <br />These monitoring wells are in support of an ongoing Environmental Assessment for Dension's <br />Sunday-Topaz Mines (CO-800-2007-104EA), but in no way approves any of the proposed <br />actions or alternatives in that EA. <br />2.0 CATEGORICAL EXCLUSION <br />See the attached Categorical Exclusion form. <br />3.0 LAND USE PLAN <br />Denison Topaz Monitoring Wells CX <br />Page 1 of 5
