REPORT OF UNDESIRABLE EVENT <br />DATE/TIME OCCURRED: 3/30/2011 <br />LOCATION: 7H Pipe Line <br />WELL/FACILITY NUMBER: 7H-R <br />SURFACE OWNERSHIP: Federal <br />LEASE NUMBER: <br />TYPE OF EVENT: Blow out Fire <br />Oil Spill Saltwater Spill <br />Other (specify): Sodium bica <br />DATE/TIME REPORTED TO BLM: 3/31/2011 <br />OPERATOR: Natural Soda, Inc. <br />Indian Fee State <br />Fatality Injury Property Damage <br />Toxic Fluid Spill Gas Venting <br />rbonate -barren liquor <br />CAUSE OF EVENT: While pressure testing recovery line a pipe line gasket blew out. <br />VOLUMES OF POLLUTANT: Discharged/consumed: Approximately 500 to 1000gallons <br />Recovered: None <br />TIME REQUIRED TO CONTROL EVENT (in hours): 1 Hour <br />ACTION TAKEN TO CONTROL EVENT, DESCRIPTION OF DAMAGE, CLEAN UP PROCEDURES AND DATES: <br />The leak was noticed almost immediately upon the commencement of pressure testing. The pressure <br />was removed by closing an isolation valve to the pipeline and fixing the leak. Sodium bicarbonate <br />residue is evident along the spill path. The total area affected by the spill is estimated at 1,120 ft2. A <br />small pool at the end of the spill path has been removed and the spill path has been cleaned up. And the <br />Sodium Bicarbonate enriched Soil has been disposed of in the onsite dumpster. <br />CAUSE AND EXTENT OF PERSONAL INJURY: None <br />OTHER FEDERAL, STATE, AND LOCAL GOVERNMENT AGENCIES NOTIFIED: Colorado DRMS <br />ACTION TAKEN TO PREVENT RECURRENCE: A new procedure to test lines that have been left dormant <br />has been instituted.
