Pineda <br />Director <br />Re: Line Camp Pit, File NoM-2001-001, Receipt of 112 Construction Materials Reclamation <br />Permit Amendment Application Package <br />Dear Mr. Barton: <br />On March 23, 2011, the Division of Reclamation, Mining and Safety received your 112 Construction <br />Materials Permit Application package for the Line Camp Pit, File No. M-2001-001. Preliminary review of <br />the information received determined that the following items must be received before the Division can <br />consider your application as being submitted and technical review can begin: <br />APPLICATION FORM: The application form has conflicting information relating to the amount <br />of acreage proposed as being added to the current permit. The front page indicates that 6.8 acres <br />will be added, whereas, on page 4 of the application, it states that 7+ acres will be added to the <br />existing permit. Please clarify this discrepancy. <br />OTHER EXHIBITS: Numerous exhibits are not included in the application package. Although, it <br />is assumed that exhibits such as exhibit H and K are likely to be the same as those submitted with <br />the original permit application, please indicate whether there are any proposed changes to any of <br />the exhibits not already addressed in the amendment application materials. <br />EXHIBIT L - Reclamation Costs (Rule 6.4.12): (1) All information necessary to calculate the <br />costs of reclamation must be submitted and broken down into the various major phases of <br />reclamation. You must provide sufficient information to calculate the cost of reclamation that <br />would be incurred by the state. <br />EXHIBIT M - Other Permits and Licenses (Rule 6.4.13): A statement identifying which of the <br />fallowing permits, licenses and approvals the Operator/Applicant holds or will be seeking in order <br />to conduct the proposed mining and reclamation operations: effluent discharge permits, air quality <br />emissions permits, radioactive source material licenses, the State Historic Preservation Office <br />clearance, disposal of dredge and fill material (404) permits, permit to construct a dam, well <br />permits, highway access permits, U.S. Forest Service permits, Bureau of Land Management <br />permits, county zoning and land use permits, and city zoning and land use permits. <br />Office of Office of <br />Mined Land Reclamation Denver • Grand Junction • Durango Active and <br />Inactive Mines
