A bound copy of the NOI with three attached plates <br />2. An unbound copy of the NOI with three attached plates <br />3. A digital copy of the NOI with three attached plates <br />4. A copy of the letter submitted to Saguache County Board of Commissioners. <br />5. A copy of the letter to The USFS Geologist submitting the USFS Plan of Operations and <br />the FEDEX receipt indicating shipment of the Plan of Operations to the USFS office in <br />Monte Vista, CO. <br />6.
