PERMIT #: M-2001-107 <br />INSPECTOR'S INITIALS: MAC <br />INSPECTION DATE: March 21, 2011 <br />1) The Operator has not been submitting the groundwater monitoring data with the Annual Reports. <br />2) The Operator has not provided the Division with benchmark depths for groundwater. <br />3) The Operator is no longer taking measurements from all of the groundwater monitoring wells approved <br />under TR03. <br />4) The Operator has added a new set of groundwater monitoring wells in the north section of the mine, which <br />was added to the permit boundary under Amendment - 01. <br />The findings listed above have been cited as a problem and will require corrective action by the Operator. The <br />Operator must submit a Technical Revision which includes the following: <br />1) Submit an updated Mining Plan Map showing all of the groundwater monitoring locations. <br />2) Submit all past groundwater monitoring data. <br />3) Submit benchmark depths for groundwater as approved under TR03. <br />4) Submit a report demonstrating groundwater depths are being maintained within two feet of the benchmark <br />depths. If measured groundwater depths have a variance of greater than two feet, then the report should <br />include a plan for restoring groundwater levels to the benchmark depths. <br />This concluded the inspection. <br />Page 3 of 5
