The general plan and detailed design plan for the sedimentation pond for <br />the Orchard Valley Mine is contained in Section 2.05.3(6) and sheet 6 of the Orchard <br />Valley West Portals designs, Volume 6. The detailed design plan for the Reclaimed West <br />Mine Sediment Ponds is presented in Volume 8A. <br />(4)(a)(i)(A) <br />The maps, plans and cross sections for the East Mine ponds, impoundments and diversions <br />required under Colorado Rules and Regulations were prepazed by or under the direction of <br />and certified by Matthew Sakurada, P.E., a qualified professional engineer with <br />engineering experience in the design and construction of sedimentation ponds, excess spoil <br />and underground development waste facilities. Mr. Sakurada holds a B.S. and M.E. in <br />Civil Engineering and is registered in the State of Colorado, P.E. Number 15836. Mr. <br />Sakurada was assisted by Charles Leaf, PhD., P.E., a consulting hydrologic engineer, and <br />Geo-Hydro Consulting, Inc. <br />The West Mine maps, plans and cross-sections required under Colorado rules and <br />• regulations were prepazed by or under the direction of Graeme Major P.E., a consulting <br />engineer with Golder Associates. The construction of the sedimentation ponds was <br />supervised and inspected by Matthew Sakurada, P.E. <br />The reclaimed West Mine maps, plans and cross-section were prepared by or under the <br />direction of James E. Stover, Stale of Colorado, P.E. Number 19230. <br />(4)(a)(i)(B) <br />See Maps 8A-1 °J 8 and SA-16 ~ 18 the Ponds, Impoundments, and Dams Appendix in <br />Volume 8A and the Waste Disposal Appendix in Volume 6A.
