Pineda <br />Director <br />On March 30, 2011 the Division of Reclamation, Mining and Safety approved the Conversion application submitted to <br />the Division on November 5, 2010, addressing the following: <br />Conversion of the original IIOc permit to a 112c permit with the addition of 18.1 acres to make the total permit <br />area 28 acres. <br />The terms of the Conversion No. 1 approved by the Division are hereby incorporated into Permit No. M-2005-067. All <br />other conditions and requirements of Permit No. M-2005-067 remain in full force and effect. <br />The estimated liability amount of $41,385.98 exceeds the $15,000.00 performance bond currently held. If you have not <br />already done so, please submit additional bond in the amount of $26,385.98. The revision will not be final until the bond <br />is approved by the Division. You will have 365 days from the date of approval to submit the required financial warranty <br />increase. <br />The required annual fee for a 112c permit is $791.00 which will be due on the anniversary date of your permit. The <br />anniversary date for this permit is May 3. For the 2010-2011 fee period due this coming May, you will only be required <br />to submit the $323 annual fee for the original 1 l Oc permit.
