This bid <br />includes one original copy of the following required elements, as attached: <br />Attachment 1 -- Bid Schedule <br />Attachment 2 - Bid Security (5%) <br />We received Amendment No. 1 on January 8, 2009. Receipt of this is so noted on the bid form. <br />As we understand it, the scope of work includes the restoration of one mine portal, improve a <br />drainage channels, protection of several areas from erosion using check dams, plugging of <br />metal pipe spill way, reinforcement of the lining of the spillway, maintenance and repair of <br />several rock channels from drainage erosion, armoring above the mine portal, and restoring <br />vegetation in all disturbed areas using a seed mix provided by the Colorado Division of <br />Reclamation, Mining, and Safety (DRMS). <br />J3 proposes to perform this work as follows, based upon the information currently available to <br />us, at the time of this proposal and as observed during the site visit on January 7, 2009: <br />Task 1 - Mobilization and Demobilization <br />This task includes the transportation of all personnel, materials, and equipment to the <br />project site, as well as all lodging and travel costs. Upon completion of work, we will <br />remove all debris, equipment, and excess materials, as well as return all personnel and <br />equipment. <br />We will utilize a subcontract hauler to perform equipment delivery tasks. <br />Task 2 - Sediment Pond Maintenance <br />- J3 will removal of the corrugated metal pipe riser and plugging of the pipe to a depth of <br />4-foot below ground surface. We will utilize a track excavator to assist with this task <br />(holding of pipe) as well as for shallow excavating around the pipe to obtain a straight <br />cut line on the pipe. The pipe will be plugged using a Type II Portland Cement, <br />conforming to ASTM C150, as detailed. The pipe section and any other debris <br />generated from this activity will be disposed of in an approved landfill. <br />www.j3env.com iQ
