PERMIT #: M-2005-044 <br />INSPECTOR'S INITIALS: MAC <br />INSPECTION DATE: March 16, 2011 <br />INSPECTION TOPIC: Topsoil <br />PROBLEM/POSSIBLE VIOLATION: Problem: The topsoil stockpiles do not have established vegetation on them <br />and are therefore susceptible to erosion. Rule 3.1.9(1) states if topsoil is not replaced into the backfill area <br />within a time short enough to avoid deterioration of the topsoil, vegetative cover or other means shall be <br />employed so that the topsoil is protected from erosion. <br />CORRECTIVE ACTIONS: The operator shall seed the stockpiles with the seed mix that was submitted as part of <br />the approved Reclamation Plan. If no seed mix has been provided, the operator shall submit a seed mix for <br />approval as a technical revision to the permit along with the appropriate $216 revision fee to the Division by <br />the corrective action date. The Operator shall demonstrate compliance by submitting seed tags, a bill of sale or <br />photographs of seeding activities. The Operator may also request a follow up inspection. <br />CORRECTIVE ACTION DUE DATE: 4/15/11 <br />OBSERVATIONS <br />The inspection was conducted by Michael Cunningham of the Division of Reclamation, Mining and Safety <br />(Division). Kenneth Morrison of Lincoln County was also present for the inspection. The Hoffman Pit is located <br />1 mile east of Genoa, Colorado. The site is permitted for 9.9 acres and the post-mining land use is rangeland. <br />Gen Compliance With Mine Plan: <br />Mining at the Hoffman Pit is.currently progressing from north to south. The site is operated less than 180 days <br />per year; the site was not active at the time of the inspection. The disturbed area (including the truck load-out <br />area) was traversed with a GeoXT GPS unit; the total disturbed area was found to be 7.15 acres in size. The <br />Operator is generally grading the pit walls to 3H:1V. Several sections of the pit wall are near vertical and range <br />from 5' to 17' in height. <br />Sediment Control: <br />Erosion along the western permit boundary was noted; small rills are forming along the edges of the haul road <br />and stockpiles. The eroded material is contained on-site and has not affected areas outside of the permit <br />boundary. However, there is drainage channel located approximately 20' west of the western side of the <br />affected area. The Operator has constructed a small earthen berm along portions of the western edge of the <br />pit; the berm should be extended to cover the entire length of the disturbed area. The observed erosion has <br />been cited as a problem and will require corrective action by the Operator. <br />Topsoil: <br />Topsoil has been stockpiled around the perimeter of the pit. The topsoil stockpiles are stable and did not show <br />any signs of erosion; however, the topsoil stockpiles lacked vegetation. This has been cited as a problem and <br />will require corrective action by the Operator. <br />This concluded the inspection. <br />Page 2 of 4
