PERMIT #: M-2010-084 <br />INSPECTOR'S INITIALS: DMC <br />INSPECTION DATE: March 21, 2011 <br />OBSERVATIONS <br />This inspection was conducted in response to a 110c application submitted to the Division of Reclamation, <br />Mining and Safety (DRMS) on February 16, 2011. Michael Frick, representing the applicant was present during <br />the inspection. Photographs are included at the end of this report to illustrate some of the conditions <br />observed during the inspection. <br />The site is located approximately 30 miles southwest of Meeker. <br />Directions to the site: From CR 5, turn west onto CR 26 and travel 2.6 miles before turning south onto the <br />private ranch road. Proceed 2.4 miles to the site. <br />The Lost Cause Pond operation will include 9.9 affected acres. The affected area was marked by rebar stakes <br />located at the boundary corners. The purpose of the operation is to remove sediment from a previously <br />excavated stockwater pond, in order to reestablish the pond and to provide fill material for private use and <br />resale. The depth of excavation is not to exceed ten feet. Two ephemeral tributaries to Black Sulphur Creek, <br />Eureka Creek and Little Side Gulch, pass through the affected area. Both drainages were dry during this <br />inspection, and both appeared to seldom flow. It is not anticipated that groundwater will be encountered <br />during this operation. <br />Some excavation had already taken place at the site prior to this inspection, as shown in the photographs <br />following this report. The excavated material had been stockpiled along the western proposed permit <br />boundary. Water impounded in the excavated area appeared to be from snowmelt and not from <br />groundwater. The excavated area was not receiving inflow from Eureka Creek during this inspection. <br />No problems were noted during this inspection.
