PERMIT #: M-1977-342 <br />INSPECTOR'S INITIALS: PSH <br />INSPECTION DATE: March 24, 2011 <br />OBSERVATIONS <br />The Henderson Mine, File No. M-1977-342, was toured by the Division of Reclamation, Mining and Safety <br />(Division). The following Division staff members were present on the tour: Loretta Pineda - Division Director, <br />Tony Waldron - Minerals Department Supervisor, Peter Hays, Eric Scott, David Bird, Jared Ebert, Michael <br />Cunningham, Tim Cazier, Stephanie Reigh, Catherine Kraeger-Rovey - Mined Land Reclamation Board Member <br />and Edward Rovey. Mr. Bryce Romig and other employees with Climax Molybdenum Company were present <br />during the tour. The tour included observation of the underground facilities on the production level, crusher <br />and transfer station. Following the underground tour, the above ground mine support facilities including the <br />Urad site were inspected by Peter Hays, Tony Waldron and Eric Scott. with the Division. <br />During the inspection, Mr. Romig indicated the Operator is close on submitting a revised groundwater <br />monitoring plan (TR-05) and is working on a revision of the Environmental Protection Plan (EPP) (TR-04). <br />The milling facilities and operation were not observed during the inspection.
