PERMIT #: M-1986-002 <br />INSPECTOR'S INITIALS: THM <br />INSPECTION DATE: March 1, 2011 <br />OBSERVATIONS <br />This inspection was conducted as part of the normal monitoring program established by the Colorado Division <br />of Reclamation, Mining and Safety (Division). The Whitewater Quarry is located 7 miles southeast of Grand <br />Junction, Colorado off of Highway 141. The site is a 110c operation that is currently active. Peter Kearl of <br />United Companies of Mesa County accompanied the inspector during the inspection. <br />No active mining was occurring during the inspection. Evidence of past mining activity was observed. <br />The purpose of the inspection was to observe the conditions present at the site to perform a bond calculation in <br />order to process the succession of operator from Mr. Rudy Fontanari to Oldcastle SW Group, Inc. dba United <br />Companies of Mesa County. In order to complete the Succession of Operator process United must submit <br />the revised required financial warranty amount of $12,062.89 in an approvable form.
