PERMIT #: M-2002-114 <br />INSPECTOR'S INITIALS: THM <br />INSPECTION DATE: March 1, 2011 <br />OBSERVATIONS <br />This inspection was conducted as part of the normal monitoring program established by the Colorado Division <br />of Reclamation, Mining and Safety (Division). The 15 Road Pit is located 3 miles west of Fruita, Colorado off <br />of Highway 6 & 50. The site is a 112c operation that is currently active. Peter Kearl representing Oldcastle SW <br />dba United Companies of Mesa County accompanied the inspector during the inspection. <br />The pit was active during the inspection. Mining activity was primarily taking place in Mining area 4 in Phases <br />1, 2 and 3. Large stockpiles of construction material were observed in Mining Area 4 along with various <br />material processing equipment. The approved mine plan states that mining will progress through the pits <br />numerically. Mining Area 3 has not been mined to date and mining is occurring in Mining Area 4 a technical <br />revision to modify the mining plan is required in order to be in compliance with the approved mining plan. <br />The out of sequence mining is considered a problem and a Technical Revision to the Mining and/or <br />Reclamation plan(s) is due no later than May 10, 2011. <br />Reclamation of Mining Pit 2 was occurring during the inspection. An inclinometer was used to verify the re- <br />graded slopes were compliant with the requirements of Rule 3.1.5(7). <br />An above ground fuel tank was observed adjacent to Mining Pit 2 that had a liner that needed repositioning. <br />The liner was repositioned and secured by Mr. Kearl during the inspection. It is recommended that the liner <br />configuration be monitored routinely for spill containment capability. <br />In an effort to ensure that the financial warranty remains adequate to reclaim the site per the requirements of the <br />approved reclamation plan, DRMS has updated the reclamation cost estimate (copy enclosed). Therefore <br />pursuant to Sections 34-32.5-117(4) of the Colorado Land Reclamation Act, adequate financial warranty must <br />be submitted to the DRMS within 60 days of the mailing date of this report.
