Your request for an additional 30 day extension (now ninety (90) days after the <br />original decision date) is hereby granted. The new decision date for this application is April <br />25 , 2011. Please be advised that if you are unable to satisfactorily address any concerns <br />identified in the December 8 h, 2010 review letter before the new decision date, it will be <br />your responsibility to request an extension of the review period. If there are outstanding <br />issues that have not been adequately addressed prior to the end of the extended review <br />period, and no extension has been requested, the Division will deny this application. <br />Please keep in mind that the Division requires some time to review your responses to the <br />December 8"', 2010 letter.
