PERMIT #: M-2002-020 <br />INSPECTOR'S INITIALS: PSH <br />INSPECTION DATE: March 15, 2011 <br />Survey flags and paint were observed delineating the fence location. <br />The 25th Avenue Site is an 112c sand and gravel operation, permitted for 89.16 acres of total disturbance. The <br />permit was issued on May 30, 2002. No permit revisions have occurred at the site. <br />The site was previously owned by Flatiron Companies and mined by Lafarge as part of a larger mine site. The <br />previous mining activities extended to a depth of approximately 25 to 30 feet and created two open water <br />ponds separated by a berm. <br />The City of Greeley (Greeley) purchased the site to develop it as a lined water storage facility. Lafarge contract <br />mined the site for Greeley and completed mining the berm between the lakes in 2004, creating one lined <br />pond. Lafarge opted to not remove additional material from the bottom of the lake. The site was inactive <br />during the inspection. No topsoil stockpiles remain at the site. A pile of waste or overburden material was <br />observed in the northeast corner of the site. Greeley intends to continue mining the site down to bedrock <br />(approximately 60 feet) in the future. <br />The immediate plans are to use the pond for fishing as part of the "Fishing is Fun" program. The Greeley parks <br />department has constructed two gazebos at the site (northeast and southwest corners of the pond) and plans <br />to create fish habitats with logs and build paths around the lake. The site is schedule to open for fishing on <br />June 4, 2011. <br />The approved post mine land use for the site is developed water resource. Construction of a slurry wall was <br />completed in 2002 and approved by the State Engineer's Office in 2003. The centerline of the slurry wall was <br />marked by rebar at the time of the inspection. The inlet and outlet structures were completed in 2005. An <br />approved final augmentation plan has been issued for the site. <br />Overall, the site appeared stable and well maintained. No erosion features were observed. Vegetation was <br />well established on the north and south sides of the pond. The west side of the pond contained vegetation <br />and annual weeds and the east side as sparsely vegetated. The Division recommends paths for fishing access <br />be established to prevent determination of the established vegetation by foot traffic. The Operator will be <br />required to meet the revegetation standards prior to full release of the site, which might require re-seeding <br />area affected by the fishing activities.
