Annual Report - 2010 <br />Henderson Mine and Mill Reclamation Activities <br />Permit No. M-1977-342 <br />Date TR Correspondence Summary' <br />-Submitted <br />7/21/2010 TR-04 Request formal acknowledgement that Henderson has fulfilled the <br /> stipulation to analyze a representative sample of the pond foundation <br /> soils for shear strength in order to verify design assumptions regarding <br /> stability of the pond side slopes. <br />6/23/2010 TR-09 Request formal acknowledgement that the following stipulations have <br /> been fulfilled: <br /> 1. Written acceptance of the certification of the 3-Dam seepage <br /> pipelines and related facilities; <br /> 2. Written certification for the new section of pipeline and results <br /> of the related leak-down test; <br /> 3. A grouting report for the No. 2-Dam decant system; and <br /> 4. A report addressing the potential that No. 2-Dam decant system <br /> could create a pathway for seepage and piping and cause dam <br /> stability issues. <br />7/21/2010 TR-11 Request formal written acknowledgement that Henderson has satisfied <br /> the requirements of TR-I 1 concerning the alternative method for <br /> salvaging and stockpiling top soil. <br />7/21/2010 TR-13 Request formal written acknowledgement that the stipulation to collect <br /> snowpack data in the spring of each year relative to the Interceptor <br /> Canal's run-off management capacity is no longer applicable in light of <br /> the new Ultimate Canal. <br />3/22/2010 TR-14 Request formal written acknowledgement that the DRMS is in receipt of <br /> the final project report and as-builts for the Barge Return Pipeline <br /> system and that the related stipulation has been fulfilled. <br />Page 4
