PERMIT #: M-1996-008 <br />INSPECTOR'S INITIALS: GRM <br />INSPECTION DATE: March 2, 2011 <br />OBSERVATIONS <br />This inspection is done to document current conditions are the same as noted during the December 1, 2010 <br />inspection. No operator representative was present. <br />The inspector notes that no new disturbance is evident. There are approximately 2.2 acres of exposed water <br />and 3 acres of land disturbance in Phase 1 as previously reported. Banks around the pond are approximately <br />2:1 with no viable long term vegetation. Vegetation around the shoulder and flat areas is strictly voluntary <br />and includes tamarisk. Due to the time of year it cannot be determined if any other noxious weeds are <br />present. The bond calculation done on February 22, 2011 of $19,005.00 is close to the $18,921.00 held and <br />appears to be adequate to reclaim the disturbed areas at this time. <br />The permit for this site is the subject of a Mined Land Reclamation Board hearing on March 9, 2011 for <br />revocation due to the operator's inability to post a new financial warranty. <br />Page 2 of 4
