Please <br />identify in the table the site names as they are designated on Figure 4.2-2. <br />17. In Table 4.2iii, please include operational monitoring results for SW -2 and SW -5 since they are <br />part of the approved surface water monitoring program. Also, please change the table titles <br />from "monitoring well" to "surface water station" in the table. <br />18. Attached to the Division's adequacy review letter is an adequacy response letter for PR -2 from <br />the Colorado Division of Wildlife (CDOW) dated January 6, 2011. In their letter, the CDOW <br />offered comments concerning, among other items, surface water hydrology issues. The CDOW <br />requested that the sediment pond and ditches be lined and that upper diversion runoff be <br />channeled to the sediment pond, rather than channeled directly to receiving streams. Also, the <br />CDOW requested that there not be any filling of ephemeral drainages, such as is being <br />proposed for the air jig bench over culvert Cl 1. The CDOW further advised CAM that the <br />Army Corps of Engineers will need to be involved in the bench fill project. <br />All of the CDOW's requests discussed in their letter are to be considered part of the Division's <br />adequacy review of PR -2. In addition, the Division requests that CAM update the Division on <br />the ongoing issue of the reclassification of the stream segment. Please refer to the CDOW <br />letter provided in Attachment 3. <br />19. Please show on an appropriate figure the location of the mine water discharge pipe and <br />discharge point, or indicate where in the permit the information can be found. <br />20. Please provide the updated NPDES permit for the new sediment pond and the mine water <br />discharge point when the permit is obtained. <br />21. Please state in Section 2.04.5 and Section 2.04.7 whether East Salt Creek and Big Salt Wash <br />are perennial, intermittent or ephemeral streams. Also, please state in the permit text whether <br />any of the ephemeral streams within the permit area have drainage areas greater than one <br />square mile. <br />22. Rule 4.0 5.18 requires that the Division make a finding for disturbances within 100 feet of a <br />perennial stream, an intermittent stream or an ephemeral stream with a drainage area greater <br />than one square mile. First, several disturbed areas are or will be within 100 feet of East Salt <br />Creek. Please provide pertinent information in the permit text that demonstrates compliance <br />with 4.05.18. Second, please provide similar information if any of the ephemeral drainages are. <br />greater than one square mile in size. <br />Rule 2.04.9 Soils Resource Information <br />23. The baseline soils information is found in Appendix T in the permit revision application. An <br />Order 1 -2 soil survey was conducted on 50 acres, the area proposed for the new facilities. <br />According to Rule 2.04.9 (1)(c), a soils map or enlarged aerial photo of the same scale as the <br />permit area map shall be provided. The map provided (Figure 1) in Appendix T does not show <br />a scale, but instead, instructs the Division to look at Table 1 for a scale legend. In Table 1, <br />
