Amendment (AM-02) Application Package
Dear Mr. Varra:
On March 15, 2011, the Division of Reclamation, Mining and Safety (Division) received the 112 Construction
Materials Reclamation Permit Amendment (AM-02) Application package for the Dakolios Pit No. 1, File No. M-
1984-036. Preliminary review of the information received determined the following items must be received
before the Division can consider the application as being submitted and technical review can begin:
Application Form:
1. Please complete Item 1, the "Applicant/Operator or company name (name to be used on the permit)"
section on page 1 of the application form.
2. Please complete Item 1.1, the "Type of organization (corporation, partnership, etc.)" section on page 1 of
the application form. According to the Colorado Secretary of State website, Varra Companies, Inc. is listed
as a "Corporation".
3. The Applicant indicated the site name for the amendment application as "Varra Resources Project 110"
for Item 2 - Operation name on the application form. The Division's-files indica -th filP numh,e , P?
the amendment application form, M-1984-036 is assigned to "Dakolios Pit No. V. Please confirm it is the
Applicant's intent to change the name of the Operation assigned permit number M-1984-036. If the
Applicant does not intent to rename the site, please revise the application form to indicate "Dakolios Pit
No. 1" as the operation name.
4. On Page 2, Item #9, the Applicant indicates the Quarter Section as "NE". Based on Exhibit A - Legal
Description and Exhibit B - Survey Map submitted in the application package and the Division files, the
Division believes the proper location information is the "SW" Quarter Section. Please review the quarter
section selection and revise accordingly.
