Pirleda <br />Director <br />Re: New Construction Materials 112c Permit Application, Airport Pit, File M-2011-013, Completeness <br />Review. <br />Dear Mr. Balaz, <br />This office has reviewed the above-named permit application for completeness, and has found a few items <br />either unaddressed or missing. Please provide responses to each of the following items: <br />1. The operator's name appears inconsistently within the application form. Please check and re- <br />submit all necessary pages of the form so that they are all consistent with each other, and so that the <br />application reflects exactly the name that is registered with the Colorado Secretary of State. <br />Specifically, please check pages 1, 3, and 8. <br />2. Pages 5, 6, and 7 of the application form are missing. Please provide them, ensuring that they are <br />initialed where appropriate. <br />3. On page 8 of the application, there is a space to allow the applicant to affix a corporate seal, if one <br />exists. Please properly place a seal, or if one does not exist, please state "no seal." Please submit <br />the updated page 8. <br />4. Exhibit N - Source of Legal Right to Enter (Rule 6.4.14). There is no agreement or other form of <br />documentation attached which demonstrates the applicant's legal right to enter the site. Please <br />provide an agreement, contract, or other statement. <br />5. Please provide a statement from the applicant authorizing you to submit materials for this <br />application. <br />This letter lists the items needed to complete the application. Upon receipt of the necessary materials, this <br />office will send you notice that the application is complete, and technical review will begin.
