Summary:
Modification of the Consent Agenda agreement dated October 13, 2010 and the Board Order signed on
November 17, 2010 pertaining to MV-2010-028.
Corrective Action Modification:
1. The Operator must establish the correct permit boundary for M-1995-023 and delineate the boundaries in
accordance with Rule 3.1.12.
2. The Operator must submit and receive approval for a 110 Construction Materials "Reclamation Only"
application to permit and reclaim the disturbed area outside of the approved permit boundary. The 110c
application shall be submitted by June 14, 2011.
