Attachment to 2010 Annual Report <br />Item (a) of Annual Report Form - Per requirements of CCR 407-1 & 407-4 and <br />Rules 1.15(1) <br />Financial Warranty is covered by a Cash Escrow Account with the Valley <br />Bank & Trust. The account number is Valley Bank & Trust send <br />information on this account directly to your office each year. If your office <br />has not received the information, please notify me and I will make sure it is <br />sent to you. <br />Item (b) of Annual Report Form - Per requirements of CCR 407-4 and Rules _ <br />1.15(1) (c) Part (b) does not apply to this quarry permit. <br />Summary of quarry operations; <br />Actual quarry operations still encompass approximately the same acreage as <br />previously, approximately 2.5 acres. See attached map for details of activity. <br />Blue points show permit pin locations. <br />Quarry arrangement has changed very little due to the limited operations. <br />Quarry operations have been maintained but on a more limited bases <br />throughout the year. The main part of the quarry area is still on the north <br />end and has expanded slightly while the less active area on the south end has <br />shrunk a little. There is still a storage area for top soil in center of quarry <br />area.
