Hays <br />Environmental Protection Specialist <br />Division of Reclamation, Mining and Safety <br />1313 Sherman Street, Room 215 <br />Denver, CO 80203 <br />RE: Iverson Mine, File M-2011-001, Response to Incompleteness Letter <br />Dear Mr. Hays: <br />We received your letter dated January 13, 2011 indicating that the Iverson Mine 112 <br />Construction Materials Reclamation Package was deemed incomplete due to some omissions <br />on the application form. Below is a list of each of your comments related to the form followed <br />by an explanation of how each comment has been addressed. <br />1. The Division requires the Applicant to provide documentation giving the signer, Todd <br />Ohlheiser, the authority to sign on behalf of the company. The authorization document <br />may be in the form of by-laws, resolution, affidavit signed by corporate secretary, certified <br />secretary statement or a notarized statement on company letterhead. <br />Attached please find a notarized statement on company letterhead indicating that Todd <br />\ Ohlheiser is authorized to sign the application form on behalf of Lafarge West, Inc. <br />2. The Division requires corporations to affix a corporate seal on the certification page (page 8) <br />of the Construction Materials Regular 112c Operation Reclamation Permit Application Form. <br />Please submit a revised, original copy of the certification page with a corporate seal. If <br />Lafarge West, Inc. does not have a seal, either write "no seal" or draw a seal in the space <br />provided. <br />Attached please find a revised, original copy of the certification page containing the <br />corporate seal. <br />3. The Applicant did not complete the "Signed and dated" portion on the certification page <br />(page 8) of the Construction Materials Regular 112c Operation Reclamation Permit <br />Application Form. The date indicated in this section must match the date indicated in the <br />notary section of the form. <br />The attached revised, original copy of the certification page includes the date that it was <br />signed by Todd Ohlheiser. <br />1900 S.
