Director <br />11409 Business Park Circle, Suite 2000 <br />Longmont, Colorado 80SO4 <br />Re: Iverson Mine, File No. M-2011-001, Receipt of 112 Construction Materials Reclamation Permit <br />Application Package - Incomplete <br />Dear Mrs. Johnson: <br />On January 3, 2011, the Division of Reclamation, Mining and Safety (Division) received the 112 Construction <br />Materials Reclamation Permit Application package for the Iverson Mine, File No. M-2011-001. Preliminary <br />review of the information received determined the following items must be received before the Division can <br />consider your application as being submitted and technical review can begin: <br />Application Form: <br />1. The Division requires the Applicant to provide documentation giving the signer, Todd Ohlheiser, the <br />authority to sign on behalf of the company. The authorization document maybe in the form of by-laws, <br />resolution, affidavit signed by corporate secretary, certified secretary statement or a notarized statement <br />on company letterhead. <br />2. The Division requires corporations to affix a corporate seal on the certification page (page 8) of the <br />Construction Materials Regular 112c Operation Reclamation Permit Application Form. Please submit a <br />revised, original copy of the certification page with a corporate seal. If Lafarge West, Inc. does not have a <br />seal, either write "no seal" or draw a seal in the space provided. <br />3. The Applicant did not complete the "Signed and dated" portion on the certification page (page 8) of the <br />Construction Materials Regular 112c Operation Reclamation Permit Application Form. The date indicated <br />in this section must match the date indicated in the notary section of the form. <br />4. Please revise the "Applicant/Operator or Company Name" section on the certification page (page 8) to <br />indicate "Lafarge West, Inc." as the Applicant as indicated for Item #1 of the application. <br />The Division requires the Applicant/Operator name remain consistent and matches the exact name <br />registered with the Secretary of State's office ("Lafarge West, Inc.") throughout the application.
