Our initial review <br />indicates that all preliminary items required by Rule 2.08.4(5) have not been submitted. Thus, your application is incomplete <br />for the purposes of filing as of January 13, 2011. <br />In the 2010 Midterm Review Findings Document dated November 3, 2010 a total of 24 Items were found to be deficient. In <br />the MR-251 application submitted by TCC and received on January 10, 2011, the TCC addressed 13 of the 24 deficiencies. <br />Please provide responses to the remaining l ldeficiencies labeled Items 14 through 24.
