Vassar Way Director <br />Lakewood, CO 80227 <br />RE: Follow-up on unresolved problems at Gold Hill Mill; DRMS File No. M-1994-117 <br />Dear Mr. Beck, <br />The above-mentioned site has had one or more problems noted that, according to Division of <br />Reclamation, Mining and Safety (Division) records have not yet been resolved. According to our <br />records, the following problems were to be resolved on or before July 30, 2009. Listed below are <br />descriptions and corrective actions for the problems. Please resolve the corrective actions and <br />provide evidence of such resolution on or before February 15, 2011. The Operator may also <br />provide the Division with an update of the status of each corrective action along with a request to <br />extend the corrective action date. <br />Please note that if the corrective action(s) are not properly addressed, and evidence provided to <br />the Division by the date provided above, the listed problems could be escalated to the level of <br />possible violations and would then be referred to the Mined Land Reclamation Board for further <br />action. <br />1. Problem: Mill tailing discharged to locations outside of the approved pond. <br />Corrective Action: Provide maps and/or cross sections showing the location where tailings <br />were discharged to surface pits and underground mine workings. In addition, provide the <br />volume of tailing discharged at each location. <br />2. Problem: A thin layer of tailing was deposited on the out-slope of the tailing dam. <br />Corrective Action: Describe the cause of tailing deposition onto the dam and the measures <br />that will be taken to prevent tailing deposition in the future. <br />3. Problem: Incorrect financial warranty bond form on file. <br />Corrective Action: Provide a fully executed BLM bond form. <br />4. Problem: The permit boundary was inadequately marked. <br />Corrective Action: Install sturdy markers at each change in permit boundary direction. <br /> <br /> <br />Office of Office of <br />Mined Land Reclamation Denver • Grand Junction • Durango Active and Inactive Mines
