Please be advised that if you are unable to satisfactorily <br />address any concerns identified in this review before the decision date, it will be your <br />responsibility to request an extension of the review period. If there are outstanding issues <br />that have not been adequately addressed prior to the end of the review period, and no <br />extension has been requested, the Division will deny this application. <br />6.4.3 Exhibit C - Pre-Mining and Mining Plan Map(s) of Affected Lands <br />The list of adjoining surface/structure owners provided in Exhibit C of the original <br />Application submittal shows Matrix Energy, DCP Midstream and North Weld County Water <br />Conservancy District own structures or easements within 200 ft. of the affected land. The <br />revised Exhibit C-1: Existing Conditions Map does not identify the structures or easements <br />owned by these entities. Please clarify if these entities own structures, have easements or <br />both on or within 200 feet of the affected area and revise Exhibit C-1 accordingly. <br />6.4.4 Exhibit D - Mining Plan <br />2. Please indicate if fuel will be stored on site. If so, specify the location of fueling tanks and <br />indicate that there is a Spill Prevention Control and Countermeasure (SPCC) plan in place <br />and that the SPCC plan will be implemented prior to storage of fuel or oil products. This can <br />be done by submitting a letter to the Division, on company letterhead, affirmatively stating <br />that the plan has been implemented.
