4. The permit text, on pages 2.05-27 and 2.05-32, mentions that the broken up concrete <br />foundation will be buried on site as shown on Map 12. If no concrete was buried on site, please <br />revise this text to state that the concrete was removed offsite and revise or add a label to Map <br />12 that states that the concrete disposal area shown in view F is not applicable. If concrete or <br />other materials were disposed of onsite, please describe in the permit text and show on a map <br />what was buried and where it was buried. Also, please revise or add a label to Map 12 that <br />states that the mine office depiction on the map is no longer applicable. <br />5. The permit text does not specifically mention the reclamation of the chain link and barbed wire <br />fences and the gate, although page 2.05-31 mentions the removal of "other related surface <br />facilities".
