Dear Mr. Morrison,
On December 21, 2010 the Division of Reclamation, Mining and Safety (Division) received your
110 Construction Materials Reclamation Permit Application package for the Burgess Pit, File
No. M-2010-083. lljr'eliminary review of the information received determined that the following
items must be received before the Division can consider your application as being submitted and
technical review can begin:
Application Form
The Certification page, page 7 of the Application Form, was not filled out completely. Please
submit a revised copy of the Certification page with the line before the notary signature
completed. In addition, all signatures must be in blue ink. Please ensure the notary signature
is in blue ink on the revised Certification page. A copy of the Certification page, with the
required revisions highlighted, has been enclosed for your review.
Your application will not be considered submitted until the information listed above is received
and found sufficient to begin our review. A,decision date will be estabfisfed-3 d
date of receipt of all of the requested information. Additionally, if you have already published
notice you will need to republish notice, after the Division considers the application submitted.
This notice must be published once, starting within ten (10) days of the date your application is
considered submitted. We will notify you when you should initiate republication of your notice.
The final date for receiving comments is the 10th day after the publication or the next regular
business day.
