Reclamation Permit Application Package
Dear Mr. Morrison,
On December 21, 2010 the Division of Reclamation, Mining and Safety (Division) received your
110 Construction Materials Reclamation Permit Application package for the Jamaco Pit, File No.
M-2010-082. Jfrr'eliminary review of the information received determined that the following
items must be received efore the Division can consider your application as being submitted and
technical review can begin:
Application Form
1. The General Description was not completed on page 3 of the Application Form. Please
submit a revised copy of page 3 with the General Description provided.
2. The Certification page, page 7 of the Application Form, was not filled out completely. Please
submit a revised copy of the Certification page with the State and County information listed
and the line before the notary signature completed. In addition, all signatures must be in bl -
ink. Please ensure the notary signature is in blue ink on the revssed Certificat' e. A
copy of the Certification page, with the required revisions highlighted, has been enclosed for
your review.
Your application will not be considered submitted until the information listed above is received
and found sufficient to begin our review. A decision date will be established 30 days from the
date of receipt of all of the requested information. Additionally, if you have already published
notice you will need to republish notice, after the Division considers the application submitted.
This notice must be published once, starting within ten (10) days of the date your application is
considered submitted. We will notify you when you should initiate republication of your notice.
The final date for receiving comments is the 10th day after the publication or the next regular
business day.
