In accordance with the <br />requirements of 2.03.10(2), please provide the name and address of the issuing authority <br />for permit number 65. <br />c) Although not required by Rule 2.03.10 , please include a copy of the discharge permit <br />application that has been submitted to CDPHE WQCD for the NHN Mine. <br />d) The CDPHE Air Pollution Control Division (APCD) does not appear on the list. An <br />emissions permit will be required for the NHN Mine. Please add the APCD emissions <br />permit to the list of State permits. <br />e) Various permits may be required by the State Engineer's Office, Division of Water <br />Resources. Please add DWR and the requisite permits to this list. <br />4. Local Licenses and Permits <br />The application does not provide a category for local permits, although a Special Use Permit <br />will be required from Montrose County. Please add Montrose County (and any other local <br />entities, as appropriate) to the list of permits required. <br />2.03.11— Identification of Location of Public Office for Filing of Application <br />1. The telephone number provided for the DRMS office in Denver is incorrect. The number <br />should be (303) 866 -3567. For the Montrose County Annex in Nucla, the street address is <br />not shown. Please revise this section to give the correct phone number, for DRMS and the <br />street address for the Nucla annex. <br />2.03.12 — Newspaper Advertisement and Proof of Publication <br />1. The Division has received the proof of publication from the San Miguel Basin Forum. <br />Please revise the wording of this paragraph accordingly. <br />2.04 — INFORMATION ON ENVIRONMENTAL RESOURCES <br />2.04.3 — Site Description and Land Use Information <br />The proposed New Horizon North (NHN) Mine is situated in an area that over the past <br />several decades has been host to a series of surface coal mining operations. This sequence <br />of mining operations is described to varying degrees in the introductory paragraphs of <br />several sections included in the PAP. Because this history comes to bear on multiple aspects <br />of the proposed NHN Mine, the Division suggests that an alternate approach may be <br />preferable. Rather than repeating the history (or portions thereof) in numerous places within <br />the permit, we request that a more detailed history be written, presented at just one location <br />within the permit, and then referenced from the individual sections as appropriate. It may be <br />logical to present this narrative at the introduction to Section 2.04.3 — Site Description and <br />Land Use Information. Please revise and expand the thorough narrative presented in Section <br />2.04.3 (specific comments regarding the narrative are provided below) describing the coal <br />mining history of the area surrounding the proposed NHN Mine, and reference this <br />description from other sections of the permit. <br />
