Pineda <br />Director <br />4.05.9(17) - Examined at least quarterly by a qualified person designated by the operator, for <br />appearance of structural weakness and other hazardous conditions. <br />4.05.9(17) - A copy of the report shall be submitted to the Division on a quarterly basis. <br />It is noted that no maintenance item were conducted or identified and all impoundments were <br />functioning as designed at the time of the inspection with the exception of the following: <br />• The culvert at the Truck Loop Pond (001A) was repaired in October 2010. <br />If you have any questions feel free to contact me.
