Dear Mr. Humphries:
On Friday, December 10, 2010, the Division of Reclamation, Mining and Safety received the Construction Materials Regular
(112) Operation Reclamation Permit Amendment Application (AM5) package for the Bettger Pit, Permit No. M-1976-051. A
preliminary review of the information received determined that the following items must be received before the Division
can consider the application submitted and technical review can begin:

6.2.1 General Requirements
(2)(b) (Maps) must be prepared and signed by a registered land surveyor, professional engineer, or other
qualified person;
Please sign and resubmit the enclosed maps.
6.4.17 EXHIBIT Q - Proof of Mailing of Notices to Board of County Commissioners and Soil Conservation District
Provide proof that notice, of the permit application was sent to the Board of County Commissioners pursuant
to Subparagraph 1.6.2(1)(a)(ii).
6.4.18 EXHIBIT R - Proof of Filing with County Clerk and Recorder
Provide an affidavit or receipt indicating the date on which the application was placed with the local County
Clerk and Recorder for public review, pursuant to Subparagraph 1.6.2(1)(c).
