MINE ID # OR PROSPECTING ID #: M-1977-300 <br />INSPECTION DATE: 05 November 2010 INSPECTOR'S INITIALS: DB2 <br />OBSERVATIONS <br />This inspection was conducted by Tony Waldron and David Bird of DRMS with the primary purpose of <br />examining the progress that had been made on the water treatment system since the last inspection on 18 <br />October 2010. Representing the operator on the inspection were John Hamrick, Amory Quinn, and Randy <br />Whicker. Also attending the inspection were Thomas Mountfort, Environmental Compliance Supervisor for <br />Denver Water, Jason Kerstiens of Malcolm Pirnie, consultant to Denver Water, Timothy Cox of ARCADIS, <br />consultant to Denver Water, and Jonathan Caine of the U.S. Geological Survey. <br />Burial of the piping associated with the water treatment system has resumed and is nearly complete. The <br />upstream water release to Ralston Creek that delayed the completion of the burial last month (discussed in the <br />inspection report from 18 Oct 2010) was still ongoing, but through a special request to CDPHE-WQCD Cotter <br />was able to dewater the inundated trenches in late October, land apply the dewatering effluent to the retention <br />pond excavation area, thus creating dry conditions that allowed the burial activity to proceed. <br />Excavation and installation of the new sumps approved under TR -15 has also resumed following the <br />dewatering activities noted above. <br />Sampling and analytical activities approved under TR-14 continue. <br />DRMS collected two water samples during the inspection, one from surface water station BPL and one from <br />the water treatment plant "inlet raw" intake. Results are pending. <br />Cotter has attached a fitting to the mystery pipe that allows improved access for sending instruments down the <br />hole, and they have an improved cap on the end of the pipe. They also conducted a video log of the hole to <br />assess the integrity of the hole and drill casing, and to attempt to ascertain where the hole terminates. Cotter <br />provided a copy of the video log to DRMS. The video log provides indefinite information as to where the hole <br />terminates, and also indicates that the casing pipe is compromised in a number of sections. <br />During the inspection, DRMS observed that Cotter still has not started pumping and treating the mine pool as <br />ordered by the Mined Land Reclamation Board in their Order of 11 August 2010. <br />Cotter is reminded of the due date of 03 December 2010 for the spill report that is required as the corrective <br />action to Problem No. 1 noted in the inspection report of 18 October 2010.
