The Division has reviewed the additional application materials for CN-01, received on

November 5 and 10, 2010, and has determined that the application remains incomplete for the
purposes of filing. The following items must be received before the Division can consider the
application as being filed and commence the technical review. The filing deficiencies and
suggested corrections for CN-01 are illustrated on the attached documents.
1. Please see Attachment 1, which is a copy of the current application form.
a.) Please include the permit number on page 1 of the application form.
b.) Please correct the error in acreages, as illustrated on page 1 of the application form.
c.) Please complete item 12 on page 3 of the application form.
d.) Please identify Wildcat Mining Corporation as the Applicant on page 4 of the form
and provide the correct Applicant contact information. The suggested corrections,
provided on the attached application form, identify the corporate president.
However, a different corporate officer or agent for the corporation would also be
acceptable. According to the web site of the Colorado Secretary of State, the
registered agent for Wildcat Mining Corporation is National Registered Agents, Inc.,
1535 Grant Street, Denver, CO 80203.
e.) On page 8 of the form please identify the title of the signing corporate officer, please
affix the corporate seal, please ensure the notary properly identify the person who's
signature is notarized, and please ensure original signatures are provided in blue ink.
2. Please see Attachment 2, which is a copy of the public notice posted at the mine site.
a.) Please provide current Applicant contact information on the notice as required by
Rule 1.6.2(1)(b).
Please see Attachment 3, which is a copy of the cover letter for the application package
mailed to the La Plata County Clerk.
