Wild oA of RsclwwWon, <br />1197 Main Street Utying and Safety <br />Canon City, Colorado 81212 <br />Re: Rock Gulch Pit, Permit No. M-1980-041, Receipt of 112 Construction Materials Reclamation Permit <br />Application Package C/'/-0 ' <br />Dear Ms. Bellantoni: <br />On November 12, 2010 the Division of Reclamation, Mining, and Safety received your 112 Construction Materials <br />Reclamation Permit Conversion Application package for the Rock Gulch Pit, File No. M-1980-041. Preliminary review <br />of the information received determined that the following items must be received before the Division can consider your <br />application as being submitted and technical review can begin: <br />APPLICATION FORM: <br />EXHIBIT B - Index Map (Rule 6.4.2): An index map showing the regional location of the affected land and all <br />roads and other access to the area. A standard U.S. Geological Survey topographic quadrangle or equivalent is <br />acceptable. Scale criteria need not be followed for this map. It is noted that the site remains consistent with the <br />original permit application so a new map is not supplied. Please note that the DRMS map is from April of 1979 <br />and of very poor quality. DRMS will require a new index map. <br />EXHIBIT C - Pre-mining and Mining Plan Map(s) of Affected Lands (Rule 6.4.3): One or more maps may <br />be necessary to legibly portray the following information: <br />(c) the existing topography of the area with contour lines of sufficient detail to portray the <br />direction and rate of slope of the affected land; <br />(d) the total area to be involved in the operation, including the area to be mined and the area <br />of affected lands (see definition of "Affected Land"); <br />Per Rule 6.2.1(2)(b) all maps must be signed by a registered land surveyor, professional engineer, or other <br />qualified person, give date prepared and identify and outline the area which corresponds with the application. <br />Staff notes no signatures on any maps. Please clearly define permit and affected area boundaries on all <br />maps, provide a scale for the maps, and the required signatures. The scale shall not be larger than 1"=50 <br />or smaller than 1" = 660, the preferred scale for maps is 1 or 2" = 100 or 200 feet. <br />Office of Office of <br />Mined Land Reclamation Denver • Grand Junction • Durango Active and Inactive Mines
