Pineda <br />Haxtun, CO 80731 Director <br />RE: 7Y Ranch Pit #1; DRMS File No. M-2010-075; Receipt of 110 Construction Materials <br />Reclamation Permit Application Package <br />Dear Mr. Schafer, <br />On November 18, 2010 the Division of Reclamation, Mining and Safety (Division) received <br />your 110 Construction Materials Reclamation Permit Application package for the 7Y Ranch Pit <br />#1, File No. M-2010-075. Preliminary review of the information received determined that the <br />following items must be received before the Division can consider your application as being <br />submitted and technical review can begin: <br />Application Form <br />1. The Division requires the Applicant/Operator name listed throughout the application <br />(including within the notarization) to be consistent. Please revise the application form and list <br />the Applicant name as "Washington County Road & Bridge District #3" on items No. 1, No. <br />11, and on both places on Page 7. <br />2. On Page 7 of the application form, the notary has signed in the space where the signer's <br />name should be listed. The notary should sign on the line that reads "Notary Public". Thee <br />Notary may affix the seal in the space next to the Notary's signature. Please revise this page <br />of the application form accordingly. <br />6.3.7 Exhibit G - Source of Legal Right-to-Enter <br />3. The Applicant has provided a signed statement by the Landowner stating the <br />Operator/Applicant has legal right to enter and mine. The Division cannot accept the signed <br />statement unless it is acknowledged by a Notary Public.
