PERMIT #: M-1994-121 <br />INSPECTOR'S INITIALS: MAC <br />INSPECTION DATE: November 16, 2010 <br />OBSERVATIONS <br />The inspection was conducted by Michael Cunningham of the Division of Reclamation, Mining and Safety (Division). <br />Arthur Vasquez of O & A Stone, LLC. was also present for the inspection. The Vasquez Quarry No. 2 is located 3.5 <br />miles north of Lyons, Colorado. The site is permitted for 7.4 acres and the post-mining land use is industrial/commercial <br />Gen. Compliance With Mine Plan: <br />No mining was taking place at the time of the inspection. The Operator has disturbed approximately 5 acres to date. A <br />highwall has been created on the south edge of the permit boundary; the highwall is approximately 450 feet in length. All <br />overburden is placed at the west end of the permit area and will be used to backfill the highwall during final reclamation <br />to create 3H: IV slopes. <br />Reve etation: <br />The site contains the following noxious weeds: diffuse knapweed and common mullein. These weeds are listed on the <br />Colorado Noxious Weed List and therefore they must be managed. The presence of noxious weeds on the mine site is <br />considered a problem and will require corrective action by the Operator. The Division recommends the Operator consult <br />with Boulder County Open Space in creating a Weed Control Plan. The Weed Coordinator for Boulder County is Steve <br />Sauer: (303)678-6110, ssauer@co.boulder.co.us <br />Signs and Markers: <br />The mine identification sign was not posted at the entrance to the site. This is considered a problem and will require <br />corrective action by the Operator. <br />Financial Warranty: <br />The Division holds a financial warranty in the amount of $ 7,670.00, which has been determined to be sufficient to <br />complete reclamation. <br />This concluded the inspection. <br />Page 2 of 4
