PERMIT #: M-1994-121
INSPECTOR'S INITIALS: MAC
INSPECTION DATE: November 16, 2010
OBSERVATIONS
The inspection was conducted by Michael Cunningham of the Division of Reclamation, Mining and Safety (Division).
Arthur Vasquez of O & A Stone, LLC. was also present for the inspection. The Vasquez Quarry No. 2 is located 3.5
miles north of Lyons, Colorado. The site is permitted for 7.4 acres and the post-mining land use is industrial/commercial
Gen. Compliance With Mine Plan:
No mining was taking place at the time of the inspection. The Operator has disturbed approximately 5 acres to date. A
highwall has been created on the south edge of the permit boundary; the highwall is approximately 450 feet in length. All
overburden is placed at the west end of the permit area and will be used to backfill the highwall during final reclamation
to create 3H: IV slopes.
Reve etation:
The site contains the following noxious weeds: diffuse knapweed and common mullein. These weeds are listed on the
Colorado Noxious Weed List and therefore they must be managed. The presence of noxious weeds on the mine site is
considered a problem and will require corrective action by the Operator. The Division recommends the Operator consult
with Boulder County Open Space in creating a Weed Control Plan. The Weed Coordinator for Boulder County is Steve
Sauer: (303)678-6110, ssauer@co.boulder.co.us
Signs and Markers:
The mine identification sign was not posted at the entrance to the site. This is considered a problem and will require
corrective action by the Operator.
Financial Warranty:
The Division holds a financial warranty in the amount of $ 7,670.00, which has been determined to be sufficient to
complete reclamation.
This concluded the inspection.
