ameO <br />• <br />6.1.4 Defect and Repair Observations <br />AMEC inspected geomembrane panels for damage that may have occurred during <br />manufacture, transport, deployment, or installation as discussed in Section 6.1. Any <br />damage noted (during or after deployment) by AMEC was reported. AMEC repeatedly <br />observed all panels and seams for defects until Ames covered the geomembrane with DCF. <br />AMEC documented defects by assigning a unique defect identification code to each defect, <br />and marking each defect code on the geomembrane. The summaries of defects and <br />repairs, including the approximate locations of each repair, are presented in Appendix J-4 <br />for the 2010 Phase 5 OSA primary geomembrane layer. <br />6.1.5 Destructive Testing <br />AMEC selected destructive test locations based on Lone Star's completion of 9,417 lineal <br />feet of welding for the 2010 Phase 5 OSA primary geomembrane, various working <br />conditions, and on seaming observations (e.g., suspicion of a defective weld). Lone Star <br />obtained and tested 22 fusion seam destructive samples during the installation of the 80-mil <br />LLDPE geomembrane for the 2010 Phase 5 OSA. <br />Lone Star constructed approximately 9,417 lineal feet of fusion welded seams on the 2010 <br />Phase 5 OSA primary geomembrane. The specified frequency of destructive testing was <br />one test per 500 lineal feet of weld. AMEC identified and Lone Star obtained destructive <br />samples from the 2010 Phase 5 OSA primary geomembrane panel seams at a frequency of <br />one destructive test sample per 428 lineal feet of fusion welded seams. The test frequencies <br />exceeded project requirements. <br />Field destructive testing of seams consisted of testing ten 1-inch wide coupons, five of which <br />were tested for peel, and five of which were tested for shear. Lone Star tested the inside <br />and outside tracks of the fusion welds for peel. Lone Star performed all destructive tests <br />on-site using a field tensiometer, and AMEC observed and documented all tests. <br />• <br />AMEC Earth & Environmental, Inc. <br />2000 S.
