ameley <br />• Measuring geomembrane panel thickness and confirming adequate panel <br />overlap; <br />• Visually inspecting geomembrane sheet quality, sheet grinding, seam <br />alignment, and continuity; <br />• Retaining archive samples of geomembrane destructive tests and trial <br />seams; and <br />• Observation and documentation of geogrid deployment for remediation of <br />underground workings. <br />1.3 Project Coordination <br />AMEC CQC and CQA activities were directed by the AMEC Resident Engineer. The AMEC <br />Resident Engineer (CQA Supervisor) for the construction of the 2010 Phase 5 VLF, for the <br />period January 2009 to November 2010, was Mr. Joseph D. Hickey. The Certifying <br />Engineer, Mr. John F. Lupo, Ph.D., P.E. or his representative, Mr. Derek T. Wittwer, P.E., <br />Project Manager, made periodic site visits to resolve technical issues and to review AMEC <br />activities. Appendix C presents a summary of the AMEC staff on-site during the period of <br />January 2009 through November 2010. <br />Representatives of CC&V, Ames, Lone Star, and AMEC conducted daily, weekly, and <br />preconstruction coordination meetings throughout the construction period. <br />1.3.1 Daily Construction Summary Reports <br />AMEC prepared daily construction summary reports as well as photographic documentation <br />(Appendix B) throughout the project to document the key elements of construction and the <br />CQA activities that occurred. On-site AMEC CQA personnel, the Certifying Engineer and <br />the Project Engineers discussed construction activities on a regular basis. In addition, field <br />personnel prepared daily observation reports. AMEC submitted these daily summary <br />reports to CC&V. Daily reports are not included in this report but were available for <br />Colorado Division of Reclamation Mining and Safety (DRMS) review during the course of the <br />project. <br /> <br />AMEC Earth & Environmental, Inc. <br />2000 S.
